Pilates & Back Pain: Why Movement Is Often the Best Medicine

How this low-impact method can improve strength, reduce pain, and support long-term back health.

Back pain is one of the most common issues we see at NK Physio, and for many people, it lingers for months (or even years), getting in the way of work, daily tasks, exercise, or just feeling like yourself.

While rest might seem like the obvious solution, research and experience show us that movement is key to improving your pain and Pilates is a fantastic place to start.

At NK, Rachel and Heather are both physio-led Pilates instructors and use this movement daily for their rehab of clients. We’ve realised we probably don’t sing about the benefits enough and how a course of Pilates can really help your long-standing nagging back pain. So here is a bit more about it:

What is Pilates and why does it help?

Pilates is a low-impact form of exercise that focuses on:

  • Core strength
  • Postural alignment
  • Breathing
  • Controlled, mindful movement

It’s not about ‘getting great abs’ or being bendy and doing fancy poses. It’s about connecting your breath to movement, allowing your back muscles to relax, engaging the meaningful deep stabilisers that most of us don’t know how to turn on. It’s about giving you confidence to move, bend, and lift heavy objects without fear of your back just ‘going’.

For people with back pain, Pilates can be game-changing.

At NK Physio, we regularly include Pilates as part of our rehab and long-term management plans. Here’s why it works so well:

1. Core Strength That Actually Supports Your Spine

Pilates targets the deep stabilising muscles (like the transverse abdominis and multifidus), not just the surface abs. These muscles act like a natural “corset” to support your spine and reduce strain in daily life.

That may not seem like much at first… until it starts affecting your daily life.

2. Better Posture and Movement Awareness

Many back pain issues are linked to posture or movement habits built up over time. Pilates helps you notice and correct these issues, whether that’s how you sit at your desk, lift a box, or even stand in the kitchen.

3. Improved Flexibility and Mobility

When joints or muscles around your hips, spine, or shoulders are stiff, your back often compensates, and pain follows. Pilates helps restore smooth, pain-free movement, giving your spine the support it needs without overstretching or forcing positions.

4. Breathwork for Tension

Did you know most people with back pain don’t know how to breathe optimally?

Back pain often comes with muscle tension. Pilates uses breathwork to release this tension, improve core activation, and calm your nervous system, which can reduce the sensitivity of pain itself.

5. Low Impact, High Reward

Unlike high-intensity workouts, Pilates is gentle yet challenging. Movements are controlled, progressive, and easily adapted, so you can build strength without flaring up symptoms.

“But won’t movement make it worse?”

This is a really common worry, and the short answer is: not when it’s the right movement.

When back pain strikes, your body often tries to “guard” the area by tightening up. Over time, this bracing actually feeds into the pain cycle, leaving you stiff, cautious, and more uncomfortable.

Pilates helps you reintroduce safe movement in a way that’s guided and confidence-building. Many people notice they feel more mobile and often less sore within just a few sessions.

Why choose NK Physio for Pilates?

Here’s what makes our approach different:

Therapist-led sessions – Whether you work with Heather or Rachel, your Pilates will be guided by a trained professional who understands pain, injury, and rehab.

1-2-1– No getting lost at the back of a busy class and just assuming you’re doing it right. You’ll get direct feedback, hands-on corrections, and exercises adapted to you.

Part of a bigger plan – If needed, we combine it with hands-on physiotherapy, massage, or rehab to maximise results.

Confidence to move again – We don’t just give you exercises, we help you understand why your pain is there and how to take control of it.

This isn’t “generic Pilates.” It’s physio-led, tailored Pilates designed for people who want to move better, reduce pain, and build a body that lasts.

Is 1-2-1 Therapy-Led Pilates Right for You?

1-2-1 sessions with Heather or Rachel are perfect if you:

  • Struggle with ongoing back pain or stiffness
  • Feel nervous about joining a large group class
  • Want targeted guidance to move safely and effectively
  • Need support rebuilding strength after an injury or flare-up
  • Simply want to improve posture, mobility, and confidence

Or perhaps you went along to a class, and it made your back worse and you never went back!
